INSURANCE REIMBURSEMENT INFORMATION.
(The information set forth below is
a guide only. Reimbursements may vary with different insurance policies
and from state to state.)
Many types of Insurance (Private Health Insurance
such as Blue Cross, Health Net, CIGNA, Aetna, HMO's, PPO's, etc..,
Auto Accident Insurance related to a recent auto accident, and Workers
Compensation Insurance related to a recent work Injury) may reimburse
you for products purchased from our store if you have a prescription
for the item from your primary care provider. (Medical Doctor, Chiropractor,
etc..)
Private health insurance companies may reimburse
depending on the policy. Read your policy or speak with your agent
about possible coverage of orthopedic supports, braces, pillows, etc.
If you do have coverage simply visit your doctor and ask if he/she
will write a prescription for the product you are interested in.
After you purchase the product from our store,
send a copy of the physician's prescription, a CPT Code for the medical
supply (we will provide the code with the product) along with the
receipt from our store to your insurance company. If the insurance
company accepts your claim, they should send you a reimbursement check
once the claim is approved. Please note that each insurance company
is different and reimbursement rates will vary. (some will pay 70%,
80% or even 100% of the purchase price, or a price them think appropriate
for that item.) Please talk with your individual insurance company
for specifics.

ask for the CPT insurance reimbursement codes for the product in the
Special Instructions section of the online check-out form. You can

and our product receipt for reimbursement.
